To help us provide the best experience possible

we ask all new patients to complete a questionnaire. You can fill it out online or onsite before your visit. Please arrive at least 10 minutes early if you plan to complete it at the office. This form helps us get to know you better and understand your medical history and preferences. Whether you’d like noise-canceling headphones, a favorite show on Netflix, a warm blanket, or a squishy stress ball, we’ve got you covered!

* Please note that if you have severe or active gum disease (periodontitis) you will require a more a more comprehsive and detailed cleaning. A more thorough conversation will be necessary to ensure you get the best outcome.

When it’s time for your appointment, your treating doctor will greet you and have a brief chat about any concerns you have. We’ll then take a full set of X-rays, photos, and a 3D scan. You’ll come back for a comprehensive check-up, where we’ll use these records to ensure a thorough understanding of your dental health. We’ll follow this with a full clean* and stain removal, finishing up with a refreshing rinse and fluoride application while you’re seated upright.

Afterward, you and your doctor will move to the consultation room to go over all the findings and answer any questions you might have.
